Enter your email address and password.
Reset your Password
If you do not remember your password, or receive the following error message, reset your password by clicking the Reset Password link below the password field.
Enter your email address and click Request Password Reset.

The email will come from no-reply@ohio.pmpaware.net. If you do not see the email, make sure to check your junk and spam folders. If you still do not see the email, you may need to add no-reply@ohio.pmpaware.net to your safe sender list or contact your internal IT department or email service provider.
You must click the link in the email, which will take you to a 3 question identity quiz. You must pass the identity quiz in order to reset your password. An example is displayed below.
Pass Identity Quiz
If you pass the identity quiz, you will be taken to the Change Password screen where you must re-enter your email address and enter a new password, meeting the on-screen requirements. Then, click Change.
So long as your email address is correct and your password meets the requirements, you'll be taken back to the login screen where a successful message is displayed and you can login with your new password:
