The Patient History Request "search" or "query" displays a list of all AWARxE and Gateway users requesting a report on the identified patient.
To perform a Patient History Request login to PMP AWARxE and navigate to Menu > Insight > New Reports. Click on Patient History Request.
Enter the required information, noting required fields are marked with a red asterisk (*). Once all required search criteria has been entered, click Run Patient History Request.
If your search results in a patient match, a Success message will display. Return to Menu > Insight > Reports History to view your report.
If a search does not return any results, a message will display indicating that either no patient matching the search criteria could be identified or the patient was identified but no prescriptions were found. Refer to the No Results Found section below for more information. |
Viewing a Patient History Request
Once the Patient History Request is ready, search results will display in Menu > Insights > Reports History.
The Patient History Request can be expanded for more detail by clicking the “+” symbol next to the patient’s name.
If configured by the applicable PDMP, users can download the Investigative Patient Report as a PDF or CSV file by clicking Download PDF or Download CSV.
No Results Found
If search results could not be matched to any patient records, a message will display indicating no matching patient could be identified.
If search criteria matches a patient's record, but the patient does not have any prescriptions within the specified timeframe, a message will display indicating the patient was found, but no prescriptions were found.
If this occurs, the user can click Change Date Range to return to the Patient History Request page, enter a different date range, and re-run the report.
Multiple Patients Identified and Partial Search Results
If multiple patients were identified when performing a Patient History Request or partial search results were utilized and identified more than one patient, a message will display indicating more than one match was identified.
From this page, a user can click Refine Search to return to the Patient Request page, refine the search criteria, and re-run the report.
OR
Select one or more of the patients displayed and click Run Report. The request will then be sent to an admin to make a determination to either approve or reject the request. Once a determination is made by the administrator, the user will receive the results of the determination via email.
