If a Patient Request is performed using an exact patient name and multiple patient profiles match the search criteria, a message will display indicating that multiple patients have been identified.
At this point, choose one of the following options:
Option 1: Refine Your Search
Click Refine Search Criteria to return to the Patient Request page, adjust the search parameters, and submit a new patient request.
Option 2: Select Patient Group(s) and Run Report
If the correct patient is included in the results:
Select one or more of the patient groups displayed.
Click Run Report to generate the Patient Report.
What Happens Next?
Immediate Patient Report Access
When immediate access to the Patient Report is granted:
The report displays immediately after clicking Run Report.
The request may also appear in Requests Processing labeled as “Multiple Patient.”
This allows administrators to review or consolidate patient records, if needed.

Any consolidation changes apply to future reports only. Historical reports remain unchanged. No additional approval is required |
Administrative Review Required
Some PDMPs require administrative review before the Patient Report will be made available to them. When this occurs:
A banner will indicate that administrative approval is required.
The report will not appear in Requests History until PDMP admin approval is complete.
The request will appear in Requests Processing as “Multiple Patient.”
Once processing and approval are complete, the report will become available in Requests History.
Secondary Approval Requirements
Some PDMPs may even require an additional level of review beyond administrative approval.
In these cases the report will not appear in Requests History until all required approvals are completed.
Patient Profile Consolidation
If multiple patient profiles represent the same individual, they may be consolidated.
To request consolidation:
Click Make a Suggestion.
Enter a note requesting review by the PDMP administrator.
Consolidation changes do not modify historical reports. Changes apply to future reports only. |
