Program Registration Information

• To register for the Arizona CSPMP, please visit https://arizona.pmpaware.net/identities/new.
• Resources for registration can be found on the program website under “PMP Users” (https://pharmacypmp.az.gov/pmp-users)   
• Prescribers licensed by an Arizona regulatory board under Title 32, who also have an active DEA number, are required by law (A.R.S. 36-2606) to register for an Arizona CSPMP account.
• Pharmacists licensed by the Arizona Board of Pharmacy who work at either a facility with an active DEA registration or are employed by a health care insurer and possess an NPI registration are required by law (A.R.S. 36-2606) to register for an Arizona CSPMP account.
