Program Registration Information
- Prior to starting your registration, please review the information provided on the PMP site at https://portal.ct.gov/dcp/prescription-monitoring-program/how-to-register
- During registration, required supplemental documentation may be uploaded to the system directly. If you are unable to upload, documents can be submitted via email to dcp.pmp@ct.gov
- A copy of your driver’s license, passport or government issued photo identification card is required.
Program Pharmacy/Dispenser Information
- Each pharmacy, nonresident pharmacy, outpatient pharmacy in a hospital or institution, and prescribing practitioners who dispense (including veterinary facilities) shall report all required drugs and medical devices dispensed immediately upon, but in no event later than the next business day (dispensing veterinarians are permitted to report weekly) after dispensing such prescriptions into the CPMRS Data Collection website at https://pmpclearinghouse.net.
- Regardless of dispensing status, zero reports are required for all in and out of state licensed pharmacies and dispensing practitioners for days that there were no reportable drugs dispensed or the dispensing facility location was closed.
Program Gateway Integration Information
- The CPMRS can now be accessed through approved electronic health records (EHR) and pharmacy management systems (PMS) where PDMP data and NarxCare analytics will be delivered directly into your workflow. The Connecticut Prescription Monitoring Program (PMP) is pleased to announce a statewide initiative to integrate the CPMRS into approved EHR/PMS using Appriss Health’s PMP Gateway service.
