Program Registration Information

If you have an existing account but no longer have access to the login email address on your account, please contact the state administrator for assistance with updating your account. Please do not attempt to create a new account.
All DEA-licensed practitioners and all pharmacists licensed in Colorado are required to register and maintain a user account with the Colorado PDMP. This includes Colorado licensees with an out-of-state DEA license and/or practitioners and pharmacists licensed in Colorado but are practicing outside Colorado.
When entering your state license number, do not include leading zeros. DR.0012345 should select “DR” for the License Type and enter “12345” for the License Number.
Nurse Practitioners must list their RXN Prescriptive Authority license number. Do not enter an RN or APN license number.

Delegate Users:
- Users who access the PDMP on a practitioner’s or pharmacist’s behalf must register their own Delegate account. All Delegate users must have their own account. Registrations under shared email addresses are not allowed.
- Delegates are required to register under their own email address but must list their supervisor(s)’ PMP AWARxE login email address in the “Supervisor’s Email” section of their registration. Please ensure you know your supervisor(s)’ PDMP registered email address before beginning your registration.
