What is a Patient Report?
A Patient Report contains the prescription history for an individual based on dispensations submitted to the PDMP. Patient Reports are intended to aid providers in their patient assessment and medical decision-making. A standard Patient Report contains the following sections:
- Patient Information & Report Criteria
- Prescription Summary
- State Indicators
- Prescriptions Detail
- Provider & Pharmacy Details

Patient Information & Report Criteria
The Patient Information & Report Criteria section of the Patient Report will provide the patient's name, age, gender, date of birth, recent address, the date the report was generated on, and the date range used for the report.
If the Patient Request included other PMP InterConnect PDMPs, a status report for states queried will be available on the Patient Report under Status of States Queried.
If multiple records are associated to the individual, a Linked Records section will present in the resulting Patient Report. Here, users can see any variations of the patient's demographic information, such as nicknames and previous addresses.
Click the caret (v) to expand or condense the Linked Records section. Each linked patient record is assigned an ID, which presents next to dispensations associated to that record in the Prescriptions section of the Patient Report.
To return to the Patient Request page and modify search criteria, users may click Refine Search.
Prescription Summary
The Rx Summary (or Prescription Summary) section of the report provides a summary of the patient's prescription history, including the total number of prescriptions in the report, the total number of different prescribers associated with the prescriptions, the number of pharmacies from which the prescriptions were dispensed. The subsequent columns provide additional summary details for opioids and buprenorphine prescriptions in the report.
State Indicators
If enabled by the PDMP, the State Indicators section of the Patient Report provides additional indicators to aid providers in their patient assessment and medical decision-making. Please refer to State Indicators for additional information on the various alerts and flags that may present in this section.
Prescriptions Detail
The Prescriptions Detail section of the Patient Report shows details about the specific prescriptions in the patient's prescription history. The total number of prescriptions in the report, as well as the number of prescriptions that were private pay are quantified at the top.
Columns within the Prescriptions Detail section of the Patient Report provide the following details for each prescription listed: the date the prescription was filled, the date the prescription was written, the patient ID the prescription is associated with, the name of the medication, the quantity of dosage units dispensed, the number of days worth of medication supplied, an abbreviation of the prescriber name, the prescription number, the dispenser that filled the prescription, refill information, MME per day for the prescription, payment type, and the PDMP from which the prescription originated.
The number of prescriptions viewed in the Prescriptions Detail table may be adjusted to a maximum of 100 lines using the View dropdown. Sorting functionality is also available for each column within the Prescriptions Detail table.
Ordering of the columns and visibility of prescription data elements within the Prescriptions Detail table are determined by the PDMP. |
Provider & Pharmacy Details
Provider and Pharmacy Detail sections of the report provide the names, addresses, and phone numbers for prescribers and dispensers associated with the presciptions on the Patient Report.
Other Patient Report Components
If enabled by the PDMP, Patient Reports may contain additional components such as the ability to export in PDF and/or CSV, icons and indicators, custom disclaimers, etc.

- If enabled, custom disclaimers may be present at the bottom of the report.
